NW Kickers maintains a financial assistance fund to support players who may need assistance in order to continue to play soccer. Funds are administered by the Board based on certain criteria including (but not limited to) such as gross income, family size, temporary financial hardship. Forms must be received by March 31. |

| Description: |
You are eligible to be reimbursed by the Club when you purchase something for the Club that is included in the budget. If the item is not included in the budget or if more money was spent then was allotted, a vote by the Club board will take place determining if and how much should be reimbursed. All items purchases on behalf of the Kickers Soccer Club, under the directions or permissions of the Club President or Club Treasurer will be promptly reimbursed upon completion of the following form. All receipts and other documents relevant to the purchase should accompany the form. Thank You.
