Form Name: 2008 Fall Registration and Medical form 
Description: Please complete both the Registration and Medical Information forms, typing in all fields.  Print the form off and bring it to Fall preprocessing July 24, 2008.  Registration fee:  $38.   NOTE:  Completion of the form does not guarantee placement on a team.  We will make every attempt possible to place, giving preference to current club team members until July 24, 2008.

Description: Please complete the form on line and then print it out.  Fall fee:  $40/ session   $20 Uniform fee (if needed)

Description: We value your opinion.  Please complete the form, ranking 1 to 5 on most items with 1 being poor and 5 being excellent.  Print the form off and mail it in to the address on the top of the form.  Thank you. 
   
Form Name: 2009 Summer Registration and Medical Forms * 
Description: Please complete both the Registration and Medical Information forms, typing in all fields.  Print the form off and forward it onto the address on file or turn into Club Registrar.  Registration fee:   $25 tryout fee for 2009 summer.   NOTE:  Completion of the form does not guarantee placement on a team.  We will make every attempt possible to place

Form Name: Request for Financial Aid 
Description:  

NW Kickers maintains a financial assistance fund to support players who may need assistance in order to continue to play soccer.  Funds are administered by the Board based on certain criteria including (but not limited to) such as gross income, family size, temporary financial hardship.  Forms must be received by March 31.

Description: All players and parents must complete 
   
Form Name: Expense Reimbursement (xls file) (pdf to come shortly) 
Description:

You are eligible to be reimbursed by the Club when you purchase something for the Club that is included in the budget.  If the item is not included in the budget or if more money was spent then was allotted, a vote by the Club board will take place determining if and how much should be reimbursed.  All items purchases on behalf of the Kickers Soccer Club, under the directions or permissions of the Club President or Club Treasurer will be promptly reimbursed upon completion of the following form.  All receipts and other documents relevant to the purchase should accompany the form.  Thank You.
